The clinician will maintain records of online counseling services. These records can include reference notes, copies of transcripts of chat and internet communications, treatment plans, and session summaries. These records are confidential and will be maintained as required by applicable legal and ethical standards according to the American Counseling Association and the Florida Board of Behavioral Health. You have a right to these records and Rose & Honey Counseling, LLC will provide a copy or a summary of your health information, usually within 30 days of your written request. We may charge a reasonable, cost-based fee. You can ask to correct health information about you that you think is incorrect or incomplete. You can also ask for a paper copy of the informed consent, even if you have agreed to receive the informed consent electronically. I am required by law to maintain the privacy and security of your protected health information. I will let you know promptly if a breach occurs that may have compromised the privacy or security of your information. I must follow the duties and privacy practices described in this notice and give you a copy. Your information will never be shared unless you have given me explicit written consent or you are in an emergency. Emergency services need your name, address, phone number, and basic contact information and reason for contacting first responders.
